If the only form provided is in PDF format, I would suggest that the
provider of the form would expect filler-inners to print, fill in, and post
or fax the form back. I would even go so far as to suggest that they might
not appreciate a modified PDF version of their form in the email, and this
may be the reason they provided a PDF form in the first place.
