Not off by hand. I use VI and the shell. However, emacs has an SGML
major mode that supports command completion, allows you to validate your
SGML from within emacs and such. It apparently works well, but I detest
emacs. Try emailing Glen Turner to ask more about it. Furthermore, check
out http://www.nswalsh.com who is one of the members of the DocBook
team. Currently DocBook is tied with SGML and he has some good links
from that page.
Apparently Arbortext (http://www.arbotext.com - at least I think) also
produce good tools..

It's OK but it does lack some features.
